Located in the quiet, welcoming community of Bristol, South Dakota, Sun Dial Manor & Johnson Center is a not-for-profit senior care community dedicated to providing compassionate, person-centered care. For more than 50 years, we have proudly served the region by offering quality skilled nursing and assisted living services in a peaceful rural setting that truly feels like home.
Our campus includes a 37-bed skilled nursing facility and a 14-bed assisted living center, both licensed and certified by the South Dakota Department of Health and the Centers for Medicare & Medicaid Services. This smaller size allows our team to get to know each resident personally, building meaningful relationships and providing the individualized care and attention every person deserves.

A Step by Step Guide to the Admission Process
Making the decision to move a loved one into a nursing home can bring many emotions and questions. At Sun Dial Manor, we want to make this transition as comfortable and straightforward as possible for residents and their families. We’ll guide you through each step, answer your questions, and help you prepare for this new chapter.

First Contact
The process begins when you contact Sun Dial Manor to ask questions or discuss your loved one’s care needs. Our Admissions Coordinator will help you understand the process, gather basic information, and explain what to expect.

Referral & Information Gathering
We gather information about your loved one’s medical history, current health, medications, daily care needs, and insurance coverage. Medical records and other necessary documentation may be requested to help our team understand their individual needs.

Clinical Review & PASRR Screening
Our team reviews the information provided to determine whether Sun Dial Manor can safely and appropriately meet your loved one’s care needs.
A PASRR (Preadmission Screening and Resident Review) is required for every resident as part of the admission process. This screening helps identify certain mental health or intellectual and developmental disability needs and determine whether additional evaluation or services are needed.
The PASRR process typically takes 24–48 hours to complete. This timeframe may vary depending on the information provided and whether additional review is necessary.
We may contact you or your loved one’s healthcare providers for additional information before making an admission determination.

Next Steps & Arrangements
Once admission is approved, we work with you to coordinate the anticipated admission date, necessary paperwork, insurance and billing details, and any medical or care arrangements needed before arrival. Our team will explain what needs to be completed and help you prepare.

Preparing for Your Loved One’s Arrival
We’ll help you understand what to bring, how to label clothing and personal belongings, and what to expect on admission day. We also encourage families to share information about their loved one’s preferences, routines, interests, and personal history so we can get to know the person behind the care needs.

Admission Day
On arrival, our team will help your loved one settle into their room, complete necessary admission assessments and paperwork, and begin getting acquainted with their new surroundings and caregivers. We want your loved one to feel safe, comfortable, and welcomed.

Ongoing Communication & Support
Admission is just the beginning of our relationship with your family. Our team will continue learning about your loved one, communicating with you about their care, and helping you navigate questions as they arise. We believe families are an important part of each resident’s life and well-being.
